Health and well-being checks are conducted during assessments and care reviews. If needed, we can recommend a more regular schedule for these checks. As with all our care plans, this service is flexible, adjusting as needs evolve.

What is a Well-being Check?

During a routine visit, our Care Experts bring a well-being kit to monitor key health indicators, ensuring your loved one’s vital signs remain stable. Using a nationally recognized system, we provide health assessments trusted by healthcare professionals.

If anything seems unusual or requires further attention, we’ll notify you right away and, if necessary, alert a healthcare professional for immediate follow-up.

Regular Health Checks at Home

We conduct these checks right in your loved one’s home, often during assessments and care reviews, with additional checks as needed to maintain their well-being.
